HSA or High Cost-Sharing PPO: These high-deductible plans usually have the lowest premiums. However, employees will pay high out-of-pocket costs each time they visit a health care provider. In some areas, such as Los Angeles, HMO premiums may come close to these PPO levels. In an HSA-eligible plan, the employer and worker can set aside tax-sheltered funds to pay out-of-pocket medical costs; unused funds can accumulate for future use.
